Documents required


The list of documents required for first level studies and uniform Masters programmes:

  1. Original secondary school certificate (maturity) with legalization/apostille with statement (or as a separate document) stating, that the certificate entitles to apply for admission to degree programmes at universities of every type in the country in which you received the above mentioned certificate. Certified translation into Polish.

    ATTENTION! If the above mentioned education documents have no information regarding its holder's entitlements to apply for admission to degree programmes at any kind of universities in the country, such annotations should be submitted as a separate document. Certified translation into Polish is also required. That document can be issued by:

    • education authorities of the country, within which education system the certificate have been issued, or
    • accredited on the territory of the Republic of Poland diplomatic representation                      or consulate office of the country within which education system the certificate have been issued, or
    • a consul of the Republic of Poland being at the office in the country within which education system the certificate have been issued; 

  2. insurance policy against sickness and accidents for the period of the candidate’s education in Poland, the European Health Insurance Card or a declaration on accession to the Polish National Health fund (Narodowy Fundusz Zdrowia – NFZ, after arrival to Poland)
  3. English language certificate or document that confirms graduation from secondary school with English as the language of instruction.
  4. an application form signed by the candidate.
  5. passport copy/ visa copy
  6. Affidavit (one person – the same name). Certified translation into Polish.

The list of documents required for the studies on the second level:

  1. Original bachelor degree with legalization/apostille with statement (or as a separate document) stating, that the diploma entitles to apply for admission to second-cycle degree programmes at universities of every type in the country in which you received the above mentioned diploma. Certified translation into Polish.

    ATTENTION! If the above mentioned education documents have no information regarding its holder's entitlements to apply for admission to degree programmes at any kind of universities in the country, such annotations should be submitted as a separate document. Certified translation into Polish is also required. That document can be issued by:

    • education authorities of the country, within which education system the certificate have been issued, or
    • accredited on the territory of the Republic of Poland diplomatic representation or consulate office of the country within which education system the certificate have been issued, or
    • a consul of the Republic of Poland being at the office in the country within which education system the certificate have been issued;
