Documents required


The list of documents required for first level studies and uniform Masters programmes:

  1. Original secondary school certificate (maturity) with legalization (or apostille) with annotation (or as a separate document) stating, that the certificate entitles to apply for admission to degree programmes at universities of every type in the country in which you received the above mentioned certificate. Sworn translated into Polish.

ATTENTION!!! If the above mentioned education documents have no information regarding its holder's entitlements to apply for admission to degree programmes at any kind of universities in the country and aboard, such annotations should be submitted as a separate document. Sworn translation into Polish is also required. That document can be issued by:

  • education authorities of the country, within which education system the certificate have been issued, or
  • accredited on the territory of the Republic of Poland diplomatic representation or consulate office of the country within which education system the certificate have been issued, or
  • a consul of the Republic of Poland being at the office in the country within which education system the certificate have been issued; 
  1. A health certificate stating that are no objections by a foreign candidate to undertake studies in a chosen field of study (if the health certificate was issued in a foreign language, a sworn translation into Polish language is required).
  2. Photocopy of European Health Insurance Card or a photocopy of any evidence of health insurance coverage or entitlement to healthcare services.
  3. English language certificate or document that confirms graduation from secondary school with English as the language of instruction.
  4. Printed and signed application form downloaded from application system.
  5. In addition, you need to show original of your passport and visa for study purposes or residence card.
  6. Affidavit one person – the same name. Sworn translated into Polish. 

The list of documents required for the studies on the second level:

  1. Original bachelor degree with legalization (or apostille) with annotation (or as a separate document) stating, that the diploma entitles to apply for admission to second-cycle degree programmes at universities of every type in the country in which you received the above mentioned diploma. Sworn translated into Polish. 

ATTENTION!!! If the above mentioned education documents have no information regarding its holder's entitlements to apply for admission to degree programmes at any kind of universities in the country and aboard, such annotations should be submitted as a separate document. Sworn translation into Polish is also required. That document can be issued by:

  • education authorities of the country, within which education system the certificate have been issued, or
